Registration
Please fill and return the separate form. Please note: each delegate must complete a separate registration form and fax or mail it to the conference secretariat. Confirmation of registration together with acknowledgement of payment will be forwarded to each participant upon receipt of registration and payment. No confirmation will be sent before payment is received.

Replacement of registration
Participants who have pre-registered but are unable to attend may be replaced by another participant at no additional cost provided written notification to Conference Secretariat.
Cancellation of registration
In case of cancellation without replacement the fee minus 15% for handling charges will be refunded provided a written request is received by Conference Secretariat before 10th of October 2002.
Accommodation
Organising Committee has blocked a large number of rooms at hotels of different price categories (exact information in registration form).
The conference venue is situated in the city centre close to Central Train Station. Most of the hotels are within walking distance. It is recommended to make hotel bookings as soon as possible. Participants are advised to make their reservations through Conference Secretariat. See enclosed selection of hotels available for conference delegates at preferred rate.
Insurance
The organiser does not accept responsibility for individual medical, travel or personal insurance. Participants are advised to take out their own personal insurance policies.
Social programme
Welcome dinner, Gala dinner, Half-day sightseeing tour of Warsaw (for foreign participants).

Any company or organisation interested in sponsorship of the conference is requested to contact the secretariat.
