The conference will be held in Polish and English simultaneous translation will be provided. Symposium on Health Insurance on May 14th in Polish language only.

Conference location
The scientific and organisation meetings will take place in Hotel Bristol inWarsaw, located close to the Old Town.
Conference language
The conference on 12th - 13th of May will be held in English and Polish. Simultaneous translation will be
provided.OnMay 14th the Symposium on Health Insurance will be held only in Polish.
Registration
Please fill and return the separate form. Please note: each delegate must complete a separate registration form and fax or mail it to the conference secretariat. Confirmation of registration and acknowledgement of payment will be forwarded to each participant upon receipt of registration and payment. No confirmation will be sent before payment is received.
Replacement of registration
Participants who have registered but are unable to attend may be replaced by another participant at no
additional cost provided written notification to Conference Secretariat.
Cancellation of registration
In case of cancellation without replacement the fee minus 15% for handling charges will be refunded provided a
written request is received by Conference Secretariat before April 30th, 2008.
Accommodation
Organising Committee has blocked several rooms at hotels of reasonable price categories. The conference venue
is situated close to the Old Town. The recommended hotels are within walking distance. It is appreciated to make
hotel bookings as soon as possible. Participants are advised to make their reservations through Conference
Secretariat. See enclosed selection of hotels available for conference delegates at preferred rates.
Travel
Warsaw has its own international airport in close vicinity to the city centre (car / bus transport ca 30 min). There
are ICtrains toWarsaw Central frommost of the European capitals.
Insurance
The organiser does not accept responsibility for individual medical, travel or personal insurance. Participants are
advised to take out their own personal insurance policies.

Welcome Cocktail, Half-day excursion, Gala dinner.
